Office visits are by appointment only. We are able to accommodate same day appointments. We make every effort to honor your appointment time, however, emergencies do arise.

Please arrive 15 minutes before your scheduled appointment time. If you need to cancel or reschedule your appointment, please call the office as soon as possible.

If you will be 10 or more minutes late, please reschedule your appointment as a courtesy to the other patients waiting to see your provider.

New Patients:

When you schedule your first appointment, the receptionist will direct you to download forms to complete and bring with you on your appointment date.

Please note, it is very important to have previous medical or prenatal records sent to our office prior to your appointment to ensure the best quality care.

TeleMed

To maintain social distancing, we will continue to provide virtual appointments through a TeleMedicine (TeleMed) model for our patients when appropriate.

Details:

• These appointments will be over the phone with your Physician. If your Physician is unavailable, the visit will be done with a different Physician in the group or one of our Physician Assistants or Nurse Practicioners.
• The appointment will be billed to your insurance similar to an office visit.
• TeleMed appointments will offer a treatment plan (including prescriptions if needed) or identify the need for an in-person visit.
